Purpose. The purpose of this project was to improve the knowledge of providers in a southern Arizona federally qualified healthcare center (FQHC) homelessness outreach team about cardiopulmonary resuscitation (CPR) outcomes, to facilitate increases of advance care planning (ACP) for FQHC’s persons experiencing homelessness (PEH) population. Background. Primary care is the central setting of conversations about advanced care planning. Providers offer ACP, when possible, but no simple method exists to focus this care where it is needed. Health illiteracy surrounding CPR is well-documented and a frequent barrier to advancing the ACP conversations. Methods. This quality improvement project (QIP) was designed to improve provider knowledge around CPR outcomes in advance care planning (ACP) using a validated, educational video decision aid (VDA). Results. The CPR VDA did increase provider knowledge about CPR outcomes. Two of the three objectives were met: Outreach providers were educated; Measurement of knowledge change as a result of the intervention was completed. Conclusions. QIP was successful but there were limitations of time and sample size was small. Sustainability of project is currently under review with the QI department of organization. Dissemination of project will occur with sharing of QIP results with FQHC outreach department, organization’s QI department, at organization’s spring QI Fair event and two regional rural health conferences.Type
