Pilomatricomas are benign neoplasms of the hair follicle. Alterations in the skin overlying pilomatricomas are rarely reported, and there are no reports with overlying striae distensae. We present an unusual case of pilomatricoma with overlying striae distensae on the flank of a young man and report its clinical findings. Recognition of this unique presentation of pilomatricomas is crucial for timely diagnosis and improved patient care, as these lesions have the potential for malignant transformation into pilomatrix carcinoma. © 2024, National Society for Cutaneous Medicine.
